Sample — Meeting Prep Survey

This is a sample so you know what to expect. The real survey is in your Meeting Prep class — that is where you will submit it before every scheduled meeting.

Where to find the real survey

Go to your Meeting Prep class and open the Meeting Prep Survey before every scheduled teacher meeting. Your teacher will open your response and review it with you at the start of your meeting. Submit it once per meeting — it covers all your enrolled courses in one survey.

Only answer for courses you are enrolled in

In Part 3 (Academic Summary), complete one block for each course you are taking. Skip any course block that does not apply to you. Course 1 is required — Courses 2 through 7 are only for students enrolled in additional courses.
